Pdf Summary
The workshop reviews ultrasound-guided corticosteroid injection for carpal tunnel syndrome (CTS), focusing on anatomy, needle visualization, safety, and technique.

Ultrasound identifies the carpal tunnel’s bony boundaries—the scaphoid radially and pisiform ulnarly—and its contents, including the median nerve and nine flexor tendons. High-frequency linear probes (typically 15–18 MHz) help visualize the nerve and nearby vessels. Important variations and abnormalities include a bifid median nerve, persistent median artery, accessory tendons or muscles, synovitis, and ganglion cysts; these may alter the safest needle path.

An in-plane approach is preferred because it allows continuous needle visualization. The operator scans beforehand to choose a radial or, more commonly, ulnar entry path and avoid arteries and nerves. Under sterile conditions, the needle is advanced toward—but not through—the median nerve. Injectate is placed around the nerve, with ultrasound used to monitor its spread and separate the nerve from adjacent tissues.

Corticosteroid injection is an accepted treatment when CTS symptoms persist despite conservative measures, particularly in mild to moderate disease. Ultrasound guidance is associated with improved symptoms and nerve-conduction outcomes, fewer repeat injections, and fewer failures than landmark-guided (“blind”) injection. Blind placement can miss its target or injure the median nerve, even when the palmaris longus tendon is used as a landmark.

The material lists active infection, adverse reactions to anesthetic or steroid, uncontrolled diabetes, and immunosuppression as contraindications. It emphasizes informed consent, sterile technique, assessment of local anatomy, and documentation, including a written ultrasound-guidance report and saved images.
