Myasthenic crisis is a life-threatening complication of myasthenia gravis characterized by respiratory insufficiency. It can be triggered by respiratory infections or have no identifiable cause. Plasmapheresis is the preferred treatment and most patients can be successfully weaned from mechanical ventilation within a month. Diagnosis is based on respiratory insufficiency in patients with known MG, and neurological and lung examinations play a role. Management involves ICU care, respiratory support, and disease-modifying treatments. Early recognition and treatment are crucial for favorable outcomes. The usage of certain medications and the impact of pregnancy and COVID-19 are also discussed. Thymectomy is rarely necessary, and prognosis has improved. Psychological support and rehabilitation are important for recovery.
