In this clinical vignette, a 33-year-old secretary with numbness and tingling in her hand, worse on the right, presents to the EMG lab. The patient has a positive valence test but normal nerve conduction studies and needle EMG. Ultrasound is performed, and the cross-sectional area of the median nerve at the wrist is normal, but the wrist-forearm ratio is enlarged. This finding suggests carpal tunnel syndrome, despite normal electrophysiologic studies. The ultrasound provided additional useful information for the clinical diagnosis.
