In this clinical vignette, a 45-year-old mechanic presents with symptoms that could indicate ulnar neuropathy or C8 radiculopathy. Nerve conduction studies reveal a low amplitude ulnar motor and sensory response with some slowing of conduction velocity. EMG shows occasional fibrillation potentials and neurogenic motor unit changes in the first dorsal interosseous. Ultrasound is used to further diagnose the condition and shows a significant change in morphology at the medial epicondyle, indicating ulnar neuropathy. The prognosis is positive as the problem is localized, but would have been less favorable if the compression was at the cubital tunnel. The ultrasound complements the clinical neurophysiology and helps guide treatment decisions.
